Please use this form to enter information for the patient that requires the Pharmacy First Service.

This service is provided remotely Monday to Friday 9am to 3.30pm and medication will be sent to you by next day delivery.

If you require assistance outside this time please contact NHS111 for non-urgent medical care, or dial 999 for urgent medical assistance.

We will telephone you to discuss your symptoms initially and then if the service requires further NHS treatment we will follow this up with a video call appointment. Any medication will be posted out to you for next day delivery.

If you have any of the following symptoms, please contact 999 or seek urgent medical assistance:

  1. Signs of a heart attack - chest pain, pressure, heaviness, tightness, squeezing across chest.
  2. Signs of a stroke - face dropping on one side, can't fold both arms up, difficulty speaking.
  3. Sudden confusion - can't be sure of own name, age etc.
  4. Suicide attempt - by taking something or self-harming.
  5. Severe Breathing difficulty - not being able to get words out, choking or gasping.
  6. Heavy bleeding - spraying, pouring or enough to make a puddle.
  7. Severe injuries - after a serious accident.
  8. Seizures - shaking, jerking due to a fit or loss of consciousness.
  9. Sudden rapid swelling - of the mouth, lips, throat or tongue.
  10. Labour or childbirth - waters breaking, frequent intense cramps, baby being born.
