What is Wegovy 7.2mg?
Wegovy (semaglutide) follows a step-up dosing schedule, starting low and increasing every four weeks as your body adjusts. Until recently, the highest maintenance dose was 2.4mg once a week.
The MHRA approved a new 7.2mg maintenance dose on 12 January 2026 for adults with obesity who need extra support to keep losing weight. This dose is given as three separate 2.4mg injections taken one after another. On 14 April 2026, the MHRA also approved a single-dose 7.2mg pen, which will make this easier — though it's not yet available in UK pharmacies, so it's worth checking with our pharmacy team on current stock.
Nothing about the medicine itself has changed — it's still the same active ingredient, semaglutide, at the same dose.
The Wegovy dose escalation pathway
Doses increase roughly every 4 weeks under clinical supervision. The step up to 7.2mg requires a minimum of 4 weeks at the 2.4mg maintenance dose.
Who is Wegovy 7.2mg suitable for?
The 7.2mg dose isn't a starting point — it's for people already established on Wegovy who need a higher dose to keep progressing.
- Adults with obesity (BMI of 30 kg/m² or above)
- Patients who've reached and tolerated 2.4mg for at least 4 weeks
- People using Wegovy for weight management, under clinical supervision
- People who are overweight (BMI below 30 kg/m²) using Wegovy for weight management
- Patients taking Wegovy specifically to reduce cardiovascular risk

How effective is the 7.2mg dose?
The 7.2mg dose was tested in the STEP UP trial: a 72-week study of 1,407 adults with a BMI of 30 or above, all receiving diet and lifestyle support alongside their assigned treatment.
STEP UP reported results two ways: a treatment policy analysis (the primary, regulatory-standard measure, covering outcomes regardless of how consistently people took treatment) and a trial product analysis (outcomes among people who took treatment exactly as intended). Side effects to be aware of

Despite more people experiencing symptoms at the higher dose, permanent discontinuation due to gastrointestinal events was actually slightly lower on 7.2mg (3.2%, STEP UP trial) than on 2.4mg (4.3%, phase 3a trials) — most people who experience side effects find them manageable enough to continue. Most side effects overall were mild to moderate.
How do you switch from 2.4mg to 7.2mg?

Is Wegovy 7.2mg available on the NHS?
Not yet. Although the MHRA has approved the dose, NHS availability depends on a separate review by NICE (the National Institute for Health and Care Excellence), which typically takes six to nine months after MHRA approval. Can I go straight onto 7.2mg without trying 2.4mg first?
No. You need to have been on the 2.4mg maintenance dose for at least four weeks before your our pharmacists can consider stepping you up.
Is 7.2mg suitable if I'm overweight rather than living with obesity?
No — the 7.2mg dose is currently only approved for adults with a BMI of 30 kg/m² or above.
